The below video was released as part of the Accident Investigation Board of Norway’s part one report on its one report on 8 November 2018 collision between the Norwegian frigate Helge Ingstad and oil tanker Sola TS outside the Sture Terminal in Norway’s Hjeltefjord fjord near Bergen.
The part one report contains the results of the AIBN’s investigation of the sequence of events leading up to collision, which has shown shown that the collision was the result of a number of operational, technical, organizational and systemic factors including, among other things, the inexperience of the frigate’s bridge team.
The AIBN has submits a total of 15 safety recommendations based on the investigation. You can read a summary of the report and recommendations here.
