Overweight Frame Accident



operator went into the hold of the ship with a speedloader, landed and got a locked indication, raised the load until the cell guides were cleared, once the guides were cleared it became clear that one of the twist locks had locked outside of the corner casting this caused the load to dip which caused the pump truck to be ejected back into the ship hold.Curious to know if this still happens at other ports especially with the older speed loaders.
